What we require prior to reviewing
a Social Security Disability case
If you already applied, we will ask for
a denial letter dated within the past 60 days.
- You need to contact your treating physician and have them send you
a medical statement explaining why you can't work.
- Make a list of all of the physicians that have treated you since
you became disabled. You need their names, addresses and telephone
numbers. Dates of treatment are helpful if you have them, or the
date that you began treatment with each physician. Include any
hospitals that you have been in or any outpatient therapy that you
have received.
- Make a list of all of the types of work that you have done in the
past 15 years. Please provide a job description, not a list of your
employers. If you have worked as a plumber for 15 years but for 3
different companies, you would only list plumber, not the individual
companies.
